ABSTRACT

Resumo O mapeamento dos comportamentos frente ao uso de medicamentos informa as condições pelas quais um tratamento farmacológico é implementado. A localização da adesão à medicação como um problema clínico, evoca, no entanto, lugares bem demarcados quanto aos procedimentos para sua explicação e resolução. Nesse sentido, este trabalho busca problematizar os elementos fundantes do estudo da adesão à medicação, considerando que tal prática é hegemonicamente assimilada apenas por parâmetros científicos e biológicos, sem a inclusão de uma abordagem direcionada às especificidades históricas e culturais dos pacientes. Transferem-se, assim, a primazia da delimitação e a explicação da realidade ao próprio analista. Porém, o trabalho de campo demonstrou uma diversidade de posturas e agenciamentos quanto ao uso dos medicamentos, capaz de indicar que a geografia do cuidado não se orienta apenas a partir de um percurso linear, mas se apoia em espaços ambulantes e itinerantes, instalando-se em um campo paradoxal que mescla reprodução e criação.


Abstract The mapping of behaviors towards the use of medicines informs the conditions under which a pharmacological treatment is implemented. However, the location of medication adherence as a clinical problem evokes well-demarcated place as to the procedures for its explanation and resolution. In this sense, this work aims to problematize the founding elements of the study of medication adherence, considering that such practice is hegemonically assimilated only by scientific and biological parameters, without the inclusion of an approach directed to the patients' historical and cultural specificities. In this way, the primacy of delimitation and explanation of reality is transferred to the analyst. However, the field work showed a diversity of attitudes and management of medicines, capable of indicating that the geography of care is not oriented only from a linear path, but is supported by wandering and itinerant spaces, installing in a paradoxical field that mixes reproduction and creation.

ABSTRACT

Introduction: Glycemic decompensation in diabetes is one of the major factors for the development of chronic disease complications. Factors involved in the adequate control of diabetes include adherence to pharmacological treatment and knowledge about the disease.Methods: Cross-sectional study on the factors associated with adherence to drug treatment and knowledge about diabetes in diabetic patients treated at Hospital Universitário de Santa Maria between 2018 and 2019, based on the validated Morisky-Green test and on the Diabetes Knowledge Questionnaire.Results: A total of 201 patients diagnosed with diabetes were included, the majority (85.6%) of which had type 2 diabetes and were white (75.6%), with a mean age of 59.4 years. An association between insufficient knowledge about diabetes and patients with type 2 diabetes was observed. An association was found between patients with type 2 diabetes using insulin and non-adherence to drug treatment compared with patients with type 2 diabetes who did use insulin. The research also showed that non-adherence to drug treatment was associated with higher occurrence of hypoglycemia compared with patients who adhered to drug treatment.Conclusion: The data obtained in our study allows us to conclude that non-adherence to pharmacological treatment makes diabetes therapy more complicated and worsens the prognosis.

ABSTRACT

RESUMO Objetivo: Comparar por meio de questionários estruturados, o conhecimento sobre a doença, o manejo de colírios e a adesão ao tratamento de portadores de glaucoma pertencentes a dois públicos com nível de escolaridade e nível sócio econômico distintos. Métodos: Foi realizado um estudo transversal analítico aplicando-se questionários estruturados, com base em estudo exploratório para avaliação do nível de conhecimento dos portadores de Glaucoma em relação a doença em dois públicos diferentes: sistema único de saúde (SUS) e planos privados de saúde. Os questionários foram aplicados por médicos residentes em Oftalmologia. A amostra é composta de 202 pacientes dentre eles 100 atendidos pelo SUS e os outros 102 pacientes dos planos privados de saúde. Todos os questionários possuem termo de consentimento livre e esclarecido assinado pelo participante e pelo pesquisador responsável. Resultados: Os pacientes foram divididos em dois grupos, compostos por: 100 pacientes SUS e 102 planos de saúde privado. Os resultados revelaram que: 58,6% dos pacientes do SUS tinham escolaridade nenhuma a fundamental incompleto e 25,5% dos pacientes de convênio tinham algum nível superior); 49% do grupo SUS tinham renda com menos de 2 salários mínimos enquanto que grupo convênio apresentou 39,4% com mais de 4 salários mínimos (p<0,001); 51,5% do grupo SUS não tem gastos com compra de colírios e 67,4% do grupo convênio gasta mais de R$30,00 (p<0,001) portanto 77% do grupo SUS recebe ajuda e 52,5% do grupo convenio não recebe ajuda (p<0,001); 63,6% do grupo convenio acredita que a quantidade de instilações a mais do colírios não obtêm uma melhora do glaucoma, enquanto aproximadamente 50% do grupo SUS relata que há uma melhora com aumento das instilações ou não tem ideia (p=0,030); Ambos os grupos obtiveram um nível de conhecimento geral da doença semelhante, sem diferença estatística. Conclusão: Concluímos que, independente do nível de escolaridade e nível socioeconômico, havendo boa relação médico-paciente, além de acompanhamento orientado e próximo, é possível transmitir conhecimento adequado sobre a doença elevando o nível de adesão ao tratamento pelo paciente.


ABSTRACT Objective: Compare, through structured questionnaires, the knowledge about disease, management of eye drops and adherence to treatment of glaucoma patients disposed in two groups according to educational levels and socioeconomic levels. Methods: A cross-sectional analytical study was carried out applying structured questionnaires based on an exploratory study to assess the level of Glaucoma patients' knowledge relationated with the disease in two different audiences: the single health system (SUS) and private health plans. The questionnaires were used by doctors residents in Ophthalmology. A sample was composed of 202 patients among which 100 were attended by SUS and the others 102 patients were holders of private health plans. All questionnaires have a free and informed consent form signed by the participant and the responsible researcher. Results: Patients were divided into two groups, consisting of: 100 SUS patients and 102 private health plans. The results revealed that: 58.6% of SUS patients had incomplete elementary schooling and 25.5% of private health insurance patients had some level of higher education; 49% of the SUS group had an income with less than 2 minimum wages while the health insurance group presented 39.4% with more than 4 minimum wages (p <0.001); 51.5% of the SUS group has no spending on eye drops and 67.4% of the health insurance group spends more than R $ 30.00 (p <0.001) so, 77% of the SUS group receives financial aid and 52.5% of the health insurance group does not receive any financial support (p <0.001); 63.6% of the health insurance group believes that the bigger amount of instillations than eyedrops does not improve glaucoma, while approximately 50% of the SUS group reports that there is an improvement when increasing instillations or has no idea (p = 0.030); Both groups obtained a similar level of general knowledge of the disease, with no statistical difference. Conclusion: We conclude that regardless of educational and socioeconomic level if prevails a good doctor-patient relationship, in addition to close monitoring, it is possible to transmit adequate knowledge about the disease, increasing levels of treatment adherence.

ABSTRACT

O estudo examinou fatores sociodemográficos, clínicos e psicossociais que afetam a continuidade do tratamento e a adesão à medicação antirretroviral em mulheres nos primeiros três meses após o parto. Participaram 56 mulheres vivendo com HIV, com idades entre 18 e 43 anos, na sua grande maioria brancas e casadas. Foram utilizadas entrevistas sobre dados sociodemográficos e clínicos, exames laboratoriais e escalas psicológicas. Análises estatísticas revelaram que mais de um terço das participantes (37,5%) descontinuaram o próprio tratamento após o parto. A qualidade de vida e o apoio instrumental podem favorecer a continuidade do tratamento de HIV após o parto, e as condições de trabalho e a situação clínica dessas mulheres pode afetar a adesão após o parto. Tais resultados podem contribuir para o desenvolvimento de intervenções que favoreçam a continuidade do tratamento e adesão das mulheres no pós-parto. (AU)


This study examined sociodemographic, clinical, and psychosocial factors affecting retention in HIV care and antiretroviral adherence in women during the first three months after delivery. Participants were 56 women living with HIV, aged between 18 and 43 years, mostly white and married. We used interviews about socio-demographic and clinical data, laboratory tests, and psychological scales. Statistical analyses revealed that more than one-third of women (37.5%) discontinued their treatment after delivery. Quality of life and instrumental support may contribute to retention in HIV care after childbirth, and the working conditions and clinical status of these women may affect adherence after childbirth. These findings may contribute to the development of interventions that promote retention in HIV care and adherence during the postpartum period. (AU)


Este estudio examinó los aspectos sociodemográficos, clínicos y psicosociales que afectan la continuidad del tratamiento del VIH y adherencia a los antirretrovirales en las mujeres en los tres meses después del parto. Participaron 56 madres infectadas por VIH, con edades comprendidas entre 18 y 43 años. La mayoría eran blancas y estaban casadas. Se utilizaron entrevistas sobre datos sociodemográficos y clínicos, exámenes de laboratorio y escalas psicológicas. Los análisis estadísticos revelaron que más de un tercio de las mujeres (37,5%) interrumpieron su tratamiento después del parto. La calidad de vida y el apoyo instrumental pueden favorecer la continuidad del tratamiento del VIH después del parto, y que las condiciones laborales y la situación clínica de estas mujeres pueden afectar la adherencia después del parto. Estos resultados pueden contribuir al desarrollo de intervenciones para favorecer la continuidad del tratamiento y la adherencia de las mujeres en el posparto. (AU)

ABSTRACT

INTRODUCCIÓN: La resistencia antibiótica y una inadecuada adherencia terapéutica son fenómenos que favorecen la proliferación de la tuberculosis. Los cambios sociodemográficos nos desafían a conocer la realidad actual de la enfermedad a través de antecedentes que nos permitan contextualizar un nuevo escenario. OBJETIVO: Caracterizar el perfil biopsicosocial del paciente con tuberculosis y su relación con la adherencia terapéutica. MATERIAL Y MÉTODO: Estudio descriptivo, transversal, correlacional. Muestra de 90 pacientes tratados en 35 Centros de Salud Familiar de los Servicios de Salud de Iquique, Metropolitano Norte, Concepción y Reloncaví. RESULTADOS: los componentes biopsicosociales como edad, antecedentes de enfermedad mental, autoestima, situación sentimental, pertenencia a grupos de riesgo, alcoholismo, drogadicción y situación de calle presentaron una relación estadísticamente significación con la adherencia terapéutica. CONCLUSIONES: La caracterización biopsicosocial del paciente con tuberculosis visibiliza nuevos factores relacionados con la adherencia que deben ser considerados para una atención interdisciplinaria.


BACKGROUND: Antibiotic resistance and inadequate therapeutic adherence are phenomena that promote the proliferation of tuberculosis. Sociodemographic changes challenge us to know the real situation of the disease and allows us to contextualize a new scenario. OBJECTIVE: To characterize the biopsychosocial profile of the patient with tuberculosis and its relationship to therapeutic adherence. MATERIAL AND METHOD: Descriptive, cross-sectional, correlational study. Sample of 90 patients treated at 35 Family Health Centers from the following Chilean Public Health Services: Iquique, Metropolitan northern (Santiago), Concepción and Reloncaví. RESULTS: Biopsychosocial components such as age, history of mental illness, self-esteem, sentimental status, belonging to risk groups, alcoholism, drug addiction and homeless situation presented a statistically significant relationship with therapeutic adherence. CONCLUSIONS: The biopsychosocial characterization of the TB patient evidence a new adherence-related factors that should be considered for interdisciplinary care.

ABSTRACT OBJECTIVE: To examine the HIV care cascade among trans women and travestis in São Paulo - Brazil, the most populous city in South America. METHODS: Using data from a cross-sectional study carried out between November 2016 and May 2017 in the city of São Paulo (Divas Research). Respondent driven sampling (RDS) was used to recruit 386 transgender women and travestis who participated in a HIV risk survey and were tested for HIV. The cascade was defined as HIV prevalence, HIV diagnosed, Antiretroviral (ART) Prescription, and currently on ART. A multiple analysis model was conducted to identify the association between sociodemographics and the cascade gaps. RESULTS: Of the trans women living with HIV, 80.9% were already diagnosed, 76.6% of them had been prescribed, of which 90.3% were currently on treatment. Those who were registered in care had a higher rate of ART (aPR 2.06; 95%CI 1.09-3.88). Trans women between 31-40 years old (aPR 1.65; 95%CI 1.09-2.50) and those older than 40 (aPR 1.59; 95%CI 1.04-2.43) had higher prevalence of ART. CONCLUSIONS: Our data suggest an increase in the testing and treatment policy implementation among trans women in the city of São Paulo, although gaps have been found in the linkage to care. However, young trans women and those not registered in health care service may benefit from efforts to engage this part of the population in care to improve HIV treatment and care outcomes.

ABSTRACT Objectives: to identify the beliefs of people with Type 2 Diabetes Mellitus related to insulin use. Methods: a descriptive, cross-sectional, quantitative-qualitative study based on Theory of Planned Behavior and performed with 32 participants using insulin. The data were analyzed and grouped into categories by beliefs similarity, counted from the frequencies. Results: 118 behavioral, 60 normative and 97 control beliefs were issued. Among the behavioral beliefs, there was an advantage in keeping the diabetes under control and disadvantage, the pain of being pierced by the application of insulin. Regulations highlighted the children as referents who support the treatment. In control beliefs, it was observed that the application of insulin appears as easiness and difficulty to the treatment. Conclusions: the study will support the construction of a psychometric scale to measure the determinants of insulin use, revealing particularities for the structuring of educational interventions that contribute to adherence of the treatment verified in adequate glycemic control.


RESUMEN Objetivos: identificar las creencias de personas con Diabetes Mellitus tipo 2, relacionadas con el uso de insulina. Métodos: el estudio descriptivo, transversal y cuantitativo, fundamentado en la Theory of Planned Behavior y realizado con 32 participantes en uso de insulina. Los datos fueron analizados y agrupados en categorías por la similitud de las creencias, contabilizados a partir de las frecuencias. Resultados: se emitieron 118 creencias comportamentales, 60 normativas y 97 de control. Entre las creencias comportamentales, se destacó la ventaja en mantener la diabetes controlada y desventaja, dolor de la perforación por la aplicación de insulina. Las normativas destacaron a los hijos como referentes que apoyan el tratamiento. En las creencias de control, se observó que la aplicación de la insulina aparece como facilidad y dificultad al tratamiento. Conclusiones: el estudio subsidiará la construcción de escala psicométrica para medir los factores determinantes del uso de la insulina, desvelando particularidades para la estructuración de intervenciones educativas que contribuyan con adhesión del tratamiento verificado en el control glucémico adecuado.

Os dados foram analisados e agrupados em categorias pela similitude das crenças, contabilizados a partir das frequências. Resultados: foram emitidas 118 crenças comportamentais, 60 normativas e 97 de controle. Entre as crenças comportamentais, destacou-se vantagem em manter o diabetes controlado e desvantagem, dor da furada pela aplicação de insulina. As normativas destacaram os filhos como referentes que apoiam o tratamento. Nas crenças de controle, observou-se que a aplicação da insulina aparece como facilidade e dificuldade ao tratamento. Conclusões: o estudo subsidiará a construção de escala psicométrica para mensurar os fatores determinantes do uso da insulina, desvendando particularidades para estruturação de intervenções educativas que contribuam com adesão do tratamento verificado no controle glicêmico adequado.

ABSTRACT BACKGROUND: Nonadherence to antiretroviral therapy (ART) may lead to viral replication and development of antiretroviral resistance. OBJECTIVE: To identify the factors associated with nonadherence to ART among people living with the human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) (PLWHA). DESIGN AND SETTING: Cross-sectional study in a tertiary-level hospital in northeastern Brazil. METHODS: Intake of less than 90% of the antiretroviral drugs prescribed in the last week prior to the interview was defined as nonadherence. Intake was evaluated using a questionnaire. Descriptive and multivariate analyses were conducted on the study population, with estimation of the respective odds ratios and 95% confidence intervals. RESULTS: The prevalence of nonadherence was 28.4%. Significant associations were found regarding the following variables: age less than 35 years, smoking, sedentary lifestyle, lack of medication and lack of knowledge regarding the patient's HIV status, on the part of the patient's partner or family. CONCLUSIONS: Encouragement of adherence to antiretroviral therapy is one of the fundamental pillars of treatment for HIV-infected patients. The high proportion of nonadherence (28.4%) and the predictive factors related to this indicate that it is necessary to improve patients' adherence to antiretroviral therapy.

Resumen Introducción. En la tuberculosis multirresistente, el abandono del tratamiento constituye un grave problema de salud pública que afecta la calidad de vida de los pacientes, sus familias y la comunidad. El enfrentarlo supone una carga para los sistemas sanitarios debido a que provoca fuentes de transmisión libre en la comunidad e incrementa la prevalencia y la mortalidad. De ahí, la necesidad de investigar los factores asociados con esta situación. Objetivo. Determinar los factores de riesgo asociados con el abandono del tratamiento en pacientes con tuberculosis multirresistente en la región de Callao (Perú). Materiales y métodos. Se hizo un estudio analítico de casos y controles (80 casos y 180 controles) en tratamiento entre el 1° enero del 2010 y el 31 diciembre del 2012. Los factores se determinaron mediante regresión logística, y se calcularon los odds ratios (OR) y los intervalos de confianza (IC) del 95 %. Resultados. En el análisis multivariado se determinaron los siguientes factores de riesgo: no tener conocimiento de la enfermedad (OR=23,10; IC95%: 3,6-36,79; p=0,002); no creer en la curación (OR=117,34; IC95%: 13,57-124,6; p=0,000); no tener apoyo social (OR=19,16; IC95%: 1,32-27,77; p=0,030); no considerar adecuado el horario de atención (OR=78,13; IC95%: 4,84-125,97; p=0,002), y no recibir los resultados de laboratorio (OR=46,13; IC95%: 2,85-74,77; p=0,007). Conclusión. Los servicios de salud deben esforzarse en la determinación precoz de las condiciones que podrían convertirse en factores de riesgo, lo cual ayudaría a implementar preventivamente intervenciones efectivas, rápidas y de alto impacto.


Abstract Introduction: In the context of multidrug-resistant tuberculosis, abandonment of therapy represents a serious public health problem that affects the quality of life of patients, families, and communities. Managing this phenomenon places a burden on health systems since it causes free sources of transmission in the community, thereby increasing prevalence and mortality. Thus, there is a need to study factors associated with this problem. Objective: This study sought to identify risk factors associated with the abandonment of therapy by patients with multidrug-resistant tuberculosis in the Peruvian region of Callao. Materials and methods: We conducted an analytical case-control study (cases=80; controls=180) in patients under treatment from January 1st, 2010, to December 31, 2012. Risk factors were identified using logistic regression; odds ratios (OR) and 95% confidence intervals (CI) were calculated. Results: The multivariate analysis identified the following risk factors: Being unaware of the disease (OR=23.10; 95% CI 3.6-36.79; p=0.002); not believing in healing (OR=117.34; 95% CI 13.57-124.6; p=0.000); not having social support (OR=19.16; 95% CI 1.32-27.77; p=0.030); considering the hours of attention to be inadequate (OR=78.13; 95% CI 4.84-125.97; p=0.002), and not receiving laboratory reports (OR=46.13; 95% CI 2.85-74.77; p=0.007). Conclusion: Health services must focus on the early detection of conditions that may represent risk factors to proactively implement effective, rapid and high-impact interventions.

ABSTRACT

Abstract Background Impact of illness may vary with the medication adherence which in turn may vary with the attitude towards drugs. There is a paucity of research examining relationships between these variables. Objective To study the levels of drug attitude, adherence and its relationship with the impact of illness. Methods A total of 279 participants with mental illness in remission were assessed with socio-demographic and clinical proforma, scales like Hogan Drug Attitude Inventory (DAI), Impact of Illness Scale (IIS), and Morisky Medication Adherence Scale (MMAS) were used. Result Mean score on DAI, IIS, and MMAS were 2.38 (SD = 4.6), 25.88 (SD = 6.6), and 5.04 (SD = 2.2) respectively. On linear regression analysis (R2 = .122, DF = 2, F = 17.598, p < .001) IIS Score was statistically significant but negatively associated with the score of MMAS (p < .05) and DAI (p < .05). Discussion Impact of illness has an inverse relationship with the level of drug attitude and medication adherence. Improving drug attitude and adherence may buffer the impact of illness.

Resumen Introducción: El tratamiento anti-retroviral (TAR) es indispensable en pacientes con infección por VIH/ SIDA; suprimir la carga viral requiere de un estricto apego a éste, por compromiso del paciente. El fracaso del TAR es primordialmente por falta de adherencia, que puede ser debida a una deficiente calidad de vida y/o a variables psicológicas. Objetivo: Determinar la calidad de vida, variables psicológicas y la adherencia al TAR, en pacientes con infección por VIH/SIDA. Material y Método: Se incluyeron 160 pacientes con diagnóstico de infección por VIH/SIDA y con TAR. Se recabaron los instrumentos MOS SF-36 y VPAD-24, una encuesta demográfica, y datos clínicos. Se hicieron asociaciones cuantitativas y cualitativas entre las variables. Resultados: La adherencia al TAR estuvo asociada con evitar comportamiento depresivo y con ausencia de adicciones. El comportamiento depresivo se encontró asociado con las adicciones. Un 87% de pacientes estaba en el rango de mejor calidad de vida. Por debajo del promedio del puntaje de salud general estuvieron masculinos, con orientación sexual HSH, solteros, en la vitalidad a los ≥ 38 años, en dolor corporal y función social a tres esquemas TAR. Conclusión: La buena adherencia al TAR estuvo asociada a evitar comportamiento depresivo y a la ausencia de adicciones y no se asoció a la calidad de vida.


Background: Antiretroviral treatment (ART) is essential in HIV/AIDS patients. Suppressing viral load requires strict adherence to ART in addition to the patient's commitment to treatment. The failure of ART is mainly due to lack of adherence, which may in turn be due to poor quality of life and/or to psychological variables. Aim: To determine the quality of life and psychological variables and adherence to ART, in patients with HIV/AIDS. Material and Method: 160 patients diagnosed with HIV/AIDS and with ART were included. The MOS SF-36 and VPAD-24 instruments, a socio-demographic survey, and clinical data were collected. Quantitative and qualitative associations were made between the variables. Results: The adherence to ART was associated with avoidance of depressive behavior and with the absence of addictions. Depressive behavior associated with addictions. 87% of patients ranked in the best quality of life. Below the average of the general health score were males, with MSM sexual orientation, single, in vitality at ≥ 38 years, in corporal pain and with social function to three ART schemes. Conclusion: Good adherence to ART was associated with avoiding depressive behavior and with non-addictions and not associated with quality of life.

ABSTRACT

ABSTRACT Objective: To understand the feelings and behaviors of people being treated for Systemic Hypertension (SH) and Diabetes Mellitus (DM). Method: A qualitative study based on Grounded Theory and Symbolic Interactionism, with 27 participants in treatment for SH and DM followed up by the Family Health Strategy team. Open, axial and selective coding was performed, giving rise to three theoretical categories and the central category. Results: The daily life is explicit in the (lack of)care of the self with the chronic disease and feelings of sadness and anxiety are expressed as reasons for the lack of control of the disease. It points out that people take care of themselves because of fear of complications, reinforced the need for guidance on the use of medication and the empowerment of the chronic patient for self-care and care for the other. Final considerations: Knowing behaviors and feelings of people with SH and/or DM allows a professional performance beyond the chronic condition.


RESUMEN Objetivo: Comprender los sentimientos y comportamientos de personas en tratamiento de la hipertensión arterial sistémica (HAS) y la diabetes mellitus (DM). Método: Estudio cualitativo basado en la Teoría Fundamentada en los Datos y en el Interaccionismo Simbólico, con 27 participantes en tratamiento de la HAS y DM, y acompañados por el equipo Estrategia Salud de la Familia. Se procedió a la codificación abierta, axial y selectiva que originó las tres categorías teóricas y la categoría central. Resultados: El cotidiano de la vida está explícito en el (des) cuidado de sí con una enfermedad crónica. Los sentimientos de tristeza y ansiedad se expresan como motivos condicionantes para el descontrol de la enfermedad. Se señala que las personas se cuidan movidas por el miedo a las complicaciones. Se reforzó la necesidad de orientación sobre el uso de la medicación y del empoderamiento del paciente crónico para el autocuidado y cuidado del otro. Consideraciones finales: Conocer los comportamientos y sentimientos de las personas con HAS y/o DM permite una actuación profesional más allá de la condición crónica.

Método: Estudo qualitativo embasado na Teoria Fundamentada nos Dados e no Interacionismo Simbólico, com 27 participantes em tratamento para HAS e DM acompanhados pela equipe Estratégia Saúde da Família. Procedeu-se a codificação aberta, axial e seletiva que originaram três categorias teóricas e a categoria central. Resultados: O cotidiano de vida está explícito no (des)cuidado de si com a doença crônica e sentimentos de tristeza e ansiedade são expressos como motivos condicionantes para o descontrole da doença. Aponta que as pessoas se cuidam movidas pelo medo das complicações, reforçou a necessidade de orientação sobre o uso da medicação e do empoderamento do doente crônico para o autocuidado e cuidado do outro. Considerações finais: Conhecer comportamentos e sentimentos das pessoas com HAS e/ou DM permite uma atuação profissional além da condição crônica.

ABSTRACT Objective: to assess the relationship between inadequate functional health literacy and inadequate blood pressure control in older people with hypertension in Primary Health Care. Method: a cross-sectional study with sample calculated at 392. SAHLPA-18 tool was used for functional health literacy; blood pressure was measured; sociodemographic and clinical data were collected. Hierarchical logistic regression was used. Results: (high) inadequate blood pressure and (low) functional inadequate health literacy were present in 41.6% and 54.6% of the people, respectively. Factors associated with inadequate blood pressure were: inadequate functional health literacy, black-brown skin color, overweight-obesity, hypertension diagnosis time, non-adherence to exercise/diet, drug treatment. Schooling had no association with inadequate blood pressure Conclusion: hypertensive elderly people with inadequate health literacy were more likely to have inadequate blood pressure. Thus, health professionals need to value functional health literacy as a possible component to control blood pressure.


RESUMEN Objetivo: evaluar la relación entre alfabetismo funcional en salud inadecuada y control inadecuado de la presión arterial en personas ancianas hipertensas em la Atención Primaria de Salud. Método: estudio transversal com muestra calculada en 392. Se utilizó instrumento SAHLPA-18 para alfabetismo funcional en salud; a la presión arterial; recogidos datos sociodemográficos y clínicos. Se utilizo La regresión logística jerárquica. Resultados: La presión arterial inadecuada (alta) y el alfabetismo funcional en salud inadecuada (bajo) estaban presentes en el 41,6% y el 54,6% de las personas, respectivamente. Los factores asociados com la presión arterial inadecuada fueron: alfabetismo funcional en salud inadecuada, color parda-negra, sobrepeso-obesidad, tiempo de diagnóstico de la hipertensión, no adhesión a ejercicio/dieta, no adhesión al tratamiento medicamentoso. El nivel de escolaridad no tuvo asociación con la presión arterial inadecuada. Conclusión: las personas mayores hipertensas con alfabetismo funcional em salud inadecuada presentaron más posibilidades de tener presión arterial inadecuada. Así, los profesionales de La salud necesitan valorizar el alfabetismo funcional en salud como posible componente para controlar la presión arterial.

ABSTRACT Objective: To identify the influence of the variables sex, age, type of anesthesia, occurrence of intraoperative complications, pain and surgical complexity on the Aldrete-Kroulik Scoring System in patients in the immediate postoperative period, in the post-anesthesia care unit. Method: Non-experimental, longitudinal study, with a quantitative approach, conducted with 241 surgical patients. Descriptive analysis, Student's t-test, Pearson and Spearman Correlation Indexes and multiple linear regression were used to analyze the data. Results: Correlations between the Aldrete-Kroulik Scoring System and pain (r=-0.13; p=0.05) and surgical complexity (r=-0.12; p=0.05) were statistically significant. The type of anesthesia and pain (p<0.01) influenced the decrease of the Aldrete-Kroulik System's scores. Conclusion: Considering the vulnerability of patients in the immediate postoperative period, it is essential for the nurse to know the factors that can influence the Aldrete-Kroulik Scoring System to provide a safe and optimal post-anesthesia recovery.


RESUMEN Objetivo: Identificar la influencia de las variables género, edad, tipo de anestesia, ocurrencia de complicaciones intraoperatorias, dolor y porte quirúrgico sobre el Índice de Aldrete y Kroulik en pacientes en el posoperatorio inmediato, en la sala de recuperación posanestésica. Método: Estudio no experimental, longitudinal, de abordaje cuantitativo realizado con 241 pacientes quirúrgicos. Análisis descriptivo, prueba t de Student, Correlaciones de Pearson y Spearman y regresión lineal múltiple fueron utilizadas para analizar los datos. Resultados: Las correlaciones entre el Índice de Aldrete y Kroulik y el dolor (r=−0,13, p=0,05) y el porte quirúrgico (r=−0,12; p=0,05) fueron estadísticamente significativas. El tipo de anestesia y el dolor (p<0,01) influenciaron la disminución de los escores del Índice de Aldrete y Kroulik. Conclusión: Considerando la vulnerabilidad de los pacientes en el postoperatorio inmediato, es fundamental que el enfermero conozca los factores que pueden influir en el Índice de Aldrete y Kroulik para propiciar una recuperación posanestésica segura y de calidad.

ABSTRACT Objective: To analyze the association between the characteristics of follow-up in health services and adherence to antihypertensive medication in patients with cardiovascular disease. Method: Analytical study carried out with 270 patients suffering from hypertension and hospitalized due to cardiovascular complications. Data collection occurred between November 2015 and April 2016, involving sociodemographic variables, presence of self-reported diabetes, accessibility and use of health services, blood pressure levels and medication adherence (analyzed through the Morisky-Green Test). Results: The rate of adherence to antihypertensive therapy was 63.0%. Enrollment in the Hiperdia program had no statistical significance to medication adherence. People who attended at least between 4 and 6 nursing consultations throughout the data collection period (p = 0.02) had better adherence. Conclusion: The study's findings provide support for the reorientation of health services and their public policies towards improving adherence to antihypertensive therapeutics.


RESUMEN Objetivo: Analizar la asociación entre las características del seguimiento en servicios de salud y la adhesión al tratamiento antihipertensivo en pacientes con enfermedad cardiovascular. Método: Estudio analítico, realizado con 270 pacientes con hipertensión internados por la ocurrencia de complicación cardiovascular. La recolección de datos ocurrió entre noviembre de 2015 y abril de 2016. Se analizaron variables sociodemográficas, presencia de diabetes autorreferida, condiciones de acceso y utilización de servicios de salud, niveles de presión arterial y adhesión terapéutica a través de la prueba de Morisky-Green. Resultados: La tasa de adhesión terapéutica antihipertensiva identificada fue del 63,0%. El registro en el programa Hiperdia no presentó significancia estadística con la adhesión. Esta medida fue mejor en aquellos que asistieron entre 4 y 6 consultas de enfermería en el año (p=0,02). Conclusión: Los hallazgos proporcionan subsidios para la reorientación de los servicios de salud y sus políticas públicas para la ampliación de la adhesión terapéutica antihipertensiva.

Método: Estudo analítico, realizado com 270 pacientes com hipertensão internados pela ocorrência de complicação cardiovascular. A coleta de dados ocorreu entre novembro de 2015 e abril de 2016. Analisaram-se variáveis sociodemográficas, presença de diabetes autorreferida, condições de acesso e utilização de serviços de saúde, níveis de pressão arterial e adesão terapêutica por meio do Teste de Morisky-Green (TMG). Resultados: A taxa de adesão terapêutica anti-hipertensiva identificada foi de 63,0%. O cadastro no programa Hiperdia não apresentou significância estatística com a adesão. Esta medida foi melhor naqueles que compareceram entre 4 e 6 consultas de enfermagem no ano (p=0,02). Conclusão: Os achados fornecem subsídios para a reorientação dos serviços de saúde e suas políticas públicas para a ampliação da adesão terapêutica anti-hipertensiva.

ABSTRACT Objective: to assess antihypertensive treatment adherence and associated factors in workers from a hospital. Method: cross-sectional research, consisting of 108 workers who self-reported as being hypertensive. Associations between sociodemographic, work and health variables were assessed regarding adherence. Results: the mean age was 44.2 years, with predominance of the female sex and workers from the nursing area. Through blood pressure measurement, 25% of participants were classified as non-controlled hypertensive patients. Approximately 88% reported taking some sort of medication; however, 79.6% did not adhere to the antihypertensive treatment. In the multiple regression analysis, the independent factors for non-adherence were hypercholesterolemia (OR=8.10; p=0.024) and missing medical appointments (OR=4.06; p=0.048). Conclusion: we verified a significant percentage of non-adherence. Since hypertension and cholesterol are asymptomatic diseases that require continuous treatment, hypertensive patients have difficulties to understand the importance of adhering to the treatment, even being health professionals or working in hospitals.


RESUMEN Objetivo: evaluar la adhesión al tratamiento antihipertensivo en los trabajadores de una institución hospitalaria y los factores asociados. Método: investigación transversal, compuesta por 108 trabajadores que se autorrefirieron hipertensos. Se analizaron relaciones entre variables sociodemográficas, de trabajo y salud con la adhesión. Resultados: la edad media fue de 44,2 años, con predominio del sexo femenino y trabajadores del área de enfermería. Mediante la medición de la presión arterial, el 25 % fueron clasificados como hipertensos no controlados. Alrededor del 88 % declararon tomar algún medicamento, sin embargo, el 79,6 % no adherían a tratamiento antihipertensivo. En el análisis de regresión múltiple, los factores independientes para la no adhesión fueron la hipercolesterolemia (OR=8,10; p=0,024), así como la falta del paciente a la consulta médica (OR=4,06; p=0,048). Conclusión: se constató porcentaje de no adhesión expresivo. Debido a que la hipertensión y el colesterol alto son enfermedades asintomáticas y de tratamiento continuo, el paciente hipertenso tiene dificultades para comprender la importancia de la adhesión al tratamiento, incluso siendo un profesional de la salud o trabajando en un entorno hospitalario.

Método: pesquisa transversal, composta por 108 trabalhadores que se autorreferiram hipertensos. Foram analisadas associações entre variáveis sociodemográficas, de trabalho e saúde com adesão. Resultados: a idade média foi de 44,2 anos, com predomínio do sexo feminino e trabalhadores da área de enfermagem. Por meio da medida da pressão arterial, 25% foram classificados hipertensos não controlados. Aproximadamente 88% referiram tomar algum medicamento, porém, 79,6% não aderiam ao tratamento anti-hipertensivo. Na análise de regressão múltipla, os fatores independentes para a não adesão foram a hipercolesterolemia (OR=8,10; p=0,024) e faltar à consulta médica (OR=4,06; p=0,048). Conclusão: constatou-se percentual de não adesão expressivo. Em razão da hipertensão e o colesterol serem doenças assintomáticas e de tratamento contínuo, o paciente hipertenso tem dificuldades de compreender a importância da adesão ao tratamento, mesmo sendo um profissional da saúde ou trabalhando em ambiente hospitalar.

Summary Introduction: Diabetes therapeutic education and information by leaflets is important. This study aimed to understand the effectiveness of written information to diabetic patients, after six months, in the control of diabetes and medication adherence. Method: Non-pharmacological clinical trial. Randomized sample of diabetic patients of 65 volunteer doctors, distributed among the five health regions in Portugal. At the first appointment, patients were randomized in four groups (three intervention with validated leaflets and one control), leaflet reading being reinforced at the follow-up appointments in a 6-months period. Variables collected: HbA1c, home blood glucose, weight, waist circumference, blood pressure, cigarettes smoked, physical activity level, adherence to medication, medication, height, diabetes progression, age, sex and educational background. Descriptive and inferential statistics. Results: From the 709 patients recruited, 702 were studied in this 6-months period with no statistical differences in the baseline variables studied. After six months of intervention, the adherence to medication improved in the leaflet group (p=0.034). This was noticed in those under 65 years of age (p=0.027), with diabetes for ≤ 5 years (p=0.010), with educational background up to 4 years (p=0.030) and 9 years (p=0.006) and with HbA1c ≥ 7% at the beginning of the study. Conclusion: Interventions with leaflets handed in primary healthcare to people with diabetes type 2 can bring benefits in what concerns adherence to therapeutics, namely in younger people with a less studies.


Resumo Introdução: A educação terapêutica e a informação dada à pessoa com diabetes parece ser importante nesta doença de prevalência crescente. Estudar a efetividade da informação escrita dada ao utente diabético, após 6 meses, no controle e na adesão terapêutica. Método: Ensaio clínico não farmacológico. Amostra aleatorizada de diabéticos de 65 médicos de família voluntários, distribuídos pelas cinco regiões de Portugal continental. Na primeira consulta, as pessoas foram aleatorizadas (em grupos de intervenção com folhetos validados e grupo controle) e foi reforçada a leitura do folheto nas consultas de seguimento até 6 meses. Foram recolhidas as seguintes informações: HbA1c, glicemias em domicílio, peso, altura, perímetro abdominal, pressão arterial, cigarros fumados, atividade física praticada, adesão terapêutica, medicamentos tomados, tempo de evolução da diabetes, idade, sexo e educação. Estatística descritiva e inferencial. Resultados: Das 709 pessoas recrutadas, foram estudadas 702, sem diferenças significativas para as variáveis epidemiológicas medidas. Aos seis meses da intervenção, a adesão à terapêutica farmacológica melhorou mais no grupo que recebeu folheto (p=0,034), nas pessoas com menos de 65 anos (p=0,027), com diabetes há cinco anos ou menos (p=0,010), com formação de até quatro anos (p=0,030) e até nove anos (p=0,006) e com a HbA1c ≥ 7% no início do estudo (p=0,008). Conclusão: Folhetos dados nos cuidados de saúde primários a pessoas com diabetes tipo 2 podem beneficiar a adesão terapêutica a curto prazo, nomeadamente em pessoas mais novas e com menor formação.

ABSTRACT

Drug-related problems consist an important avoidable risk factor to the hospitalization in the general population. The increase of technologies to promote and recovery health and their use makes the design of services aimed at preventing health and drug problems, as well as their adequate management, a priority for public health. Pharmacist-led interventions are capable to optimize the use of medicines. However, it is important to know the characteristics and assessed outcomes of interventions, since, as a complex intervention, the variability between services can explain different performances. The objective of the scoping review is to explore randomized and non-randomized clinical trial, quasi-experimental and cohort studies to explore characteristics and assessed outcome of pharmacist-led interventions conducted in Brazil. This review will consider studies about pharmacist-led interventions, regardless of patient profile or health setting. Electronic searches will be performed in PubMed, Scopus, and LILACS databases with no time limit of publication. Two researchers, independently, will select registries and extract data of study and service characteristics, and outcomes measures. The findings will be presented in a narrative form including tables and figures to aid in data presentation, where appropriate.(AU)

ABSTRACT

A não-adesão à farmacoterapia foi primeiramente identificada há mais de 2000 anos e continua a ser um desafio, gerando intenso debate nos dias de hoje. Percebe-se que houve um aumento nas pesquisas que procuram elucidar a extensão, as causas e as consequências da não-adesão à farmacoterapia. No entanto, para entender a extensão de adesão é necessário compreender o seu conceito. Para tanto, é necessário esclarecer a diferença dos diversos termos utilizados em pesquisas de adesão, tais como cumprimento, adesão, concordância e persistência. Com o objetivo de elucidar essas e muitas outras questões, conduziu-se um mapeamento da literatura existente sobre adesão à farmacoterapia. Esta revisão narrativa foi conduzida sem limitação temporal, por meio de buscas realizadas nas bases de dados MEDLINE/PubMed e Scielo. Utilizaram-se os descritos em Ciências da Saúde (DeCS) (Medication Adherence e Adesão à Medicação; Patient Compliance e Cooperação do Paciente; Terminology e Terminologia); os termos MESH (drug therapy, medication adherence, medication therapy management, patient compliance, terminology); e outros termos gerais. Apesar de existir uma vasta literatura sobre o conceito de adesão à medicação, não há um consenso padrão estabelecido. Os termos compliance, adherence e concordance têm sido utilizados erroneamente como termos intercambiáveis de adesão à medicação. Alguns autores categorizam a não-adesão em intencional e não intencional; outros em repetida ou esporádica ou, ainda, em primária ou secundária. A adesão é um comportamento multifatorial complexo que é influenciado por diversos fatores, que muitas vezes são controversos e inconsistentes nas publicações. Apesar de existir um universo rico e promissor no campo de adesão à farmacoterapia, há muita complexidade e contradição. Muito do que se reporta à adesão é de certa forma fragmentado e controverso, o que dificulta a análise deste processo como um todo. (AU)


Non-adherence to medication was first noted over 2000 years ago and remains a challenge; the issue continues to generate intense debate. A surge in research in this area has helped to elucidate the extent, causes and consequences of patient non­adherence. However, how do we elucidate the extent of adherence without understanding its concept? What is the difference between the terms used in research in this field, such as compliance, adherence, concordance and persistence? A mapping of the existing literature on adherence to medication was conducted in order to answer this and other questions. This narrative review was conducted with no time limit, by searching MEDLINE/PubMed and Scielo databases. Health science descriptors (DeCS) were applied (Medication Adherence; Patient Compliance; Surveys and Questionnaires and Terminology); MESH terms (drug therapy, medication adherence, medication therapy management, patient compliance, terminology); and other terms. Although there is a vast literature on the concept of adherence to medication, no standard consensus is established. The terms compliance, adherence and concordance have been misused as interchangeable terms for adherence to medication. Some authors categorize non-adherence into intentional and unintentional; repeated or sporadic, and primary or secondary. Adherence is a complex multifactorial behavior influenced by several factors, which are often controversial and inconsistent in publications. Although there is a rich and promising universe in the field of medication adherence, there is a lot of complexity and contradiction as well. Everything that is related to adherence is somewhat fragmented and controversial, which makes it difficult to analyze its process as a whole. (AU)
